Benefits Open Enrollment for Employees of the City of Hazleton has started.

The City of Hazleton is committed to providing quality benefits for all employees and retirees. This is the time to enroll or make changes to your medical, dental, vision benefits and add, remove or increase voluntary life and voluntary disability insurance if you wish. You may also add or remove coverage for eligible dependents (spouse and dependent children up to age 26). This is the one time of year when you can make changes to your benefits without experiencing a Qualifying Life Event (marriage, divorce, birth/adoption of a child, etc.). The benefit selections you make during open enrollment become effective on January 1, 2024.

Benefits Open Enrollment Ends Friday, December 1st at noon

No enrollments or changes can be made after this date unless you have a qualifying event.

To make changes or if you have questions please call Renee or Michelle at (570) 669-5050 prior to Noon on Friday, December 1st. Please identify yourself as a City of Hazleton employee.

Creditable Coverage Disclosure Notice

Please read this notice carefully and keep it where you can find it. This notice has information about your current prescription drug coverage with City of Hazleton and about your options under Medicare’s prescription drug coverage. This information can help you decide whether or not you want to join a Medicare drug plan. If you are considering joining, you should compare your current coverage, including which drugs are covered at what cost, with the coverage and costs of the plans offering Medicare prescription drug coverage in your area. Information about where you can get help to make decisions about your prescription drug coverage is at the end of the notice.

Preventative or routine care helps us stay well or finds problems early when they are easier to treat. As a part of your health plan, you may be eligible to receive some of these preventive benefits with little to no cost sharing when using in-network providers. AFLAC Voluntary Insurance

Active employees interested in AFLAC products may reach out directly to our AFLAC representative, Leah Gorman at (484) 633-0121. AFLAC offers voluntary short-term disability, accident, critical illness, hospital, and cancer benefits that may help with out-of-pocket medical expenses that may not be covered by major medical. AFLAC benefits are 100% employee paid, by payroll deduction.
